SECTION IV: ADOPTION AND AMENDMENT
Adoption of and amendments to this Ethics Policy shall require the affirmative vote of at least a two-third’s majority of the full City Council - that is, five (5) votes.
NOW, THEREFORE, BE IT RESOLVED by the Mayor and City Council of the City of Surprise, Arizona, that:
(1) The City Council hereby adopts the ethics policy, which is attached hereto as Exhibit A and Is incorporated herein by reference.
PASSED AND ADOPTED this _________ day of ______________, 2009.

If you need a vote of at least two-third’s majority can it be in the consent item’s?
There is an ethics policy in the packet for Item #10 but it is not identified as Exhibit A

I, ______________________________________________, hereby certify that I have read the foregoing City of Surprise Ethics Policy (dated ____________) and I agree to be bound by the applicable laws and policies – including specifically, the City of Surprise Ethics Policy- to the best of my abilities.
There is no time frame to sign the certification of personal commitment.
